Stonington Institute is committed to providing quality substance abuse treatment options for adults in Southeastern Connecticut and surrounding areas.  We offer medical detox as well as residential addictions treatment for active duty and retired military service members in our North Stonington location.  Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients.

The primary role of the Case Manager is to coordinate the aftercare needs of each client based on a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual’s well-being and prioritize treatment goals and objectives. The Case Manager functions as an expert in the community and military resources and participates as an independent member of the multi-disciplinary treatment team.  The Case Manager uses that knowledge to assist clients in accessing the network of resources that will help maintain their community and military status and promote further growth to maximize functional independence. This level of case management while clients are in residential levels of care will begin to transition towards discharge as more stability is achieved in the individual’s life and as the individual moves toward successful completion of substance use and mental health treatment.

The Case Manager completes all processes, procedures, and case management activities directly related to admission to and discharge from the military rehabilitation program. The Case Manager also works collaboratively with clinicians, clients, military and community agencies as needed, with the goal of linking clients to appropriate continuing care resources. The Case Manager assures that case management is individualized and assures that services integrate both substance use and mental health issues (as applicable in each case based on diagnosis) and access a comprehensive continuum of services which promote recovery and assist the client to fully integrate into military life and society as a healthy, substance-free individual. The Case Manager acts as a member of a multi-disciplinary team of treatment providers, and serves as an advocate for the client, assuring that the program will fit the client rather than the client needing to fit the program.

Job Requirements
Bachelor’s degree required. Master’s in the field of mental health counseling and/or substance use ideal. Knowledge of case management processes required, military protocol strongly preferred. At least one (1) year experience working with the service population as it relates to facilitating client psycho-educational groups is strongly preferred. Valid driver’s license and driving history acceptable to the organization per policy. Passenger endorsement (PSL) preferred.
